What data do we collect?

Noreside Physio collects the following data:

·         Personal identification information: (Name, DOB, email address, phone numbers)

·         Medical Information (Presenting complaint, Previous medical history, Referring consultant/doctor/healthcare professional)

How do we collect your data?

·         By Phone (during scheduling of your appointment/follow-up appointments)

·         Online (via cliniko online booking system)

·         In person (during your consultation)

How will we use your data?

Noreside Physio uses your information in order to:

·         Adequately provide you with care with respect to your current condition and previous medical history.

·         Process your appointments, payments and referrals to third parties (orthopaedic consultants, GP or other third party healthcare professional with your prior consent only)

·         Email/text/call you with respect to your appointment

·         Email/text/call you with respect to your recommended rehabilitation and management of your condition.

·         Email/text/call you in order to follow-up on your progress.

·         Email/text you with clinic updates and information that you may find useful regarding Noreside Physio

How do we store your data?

Noreside Physio stores your data on two online systems; Cliniko (for appointments, reminders, invoices and receipts) and Physitrack (for your digital exercise prescription). All information will be kept for a minimum of 7 years following your final appointment/correspondence with us. Upon the elapse of this time period, your data will be deleted. None of your data will be shared with any third party (unless explicitly consented to by yourself in case of medical referral).
Some of your information (email address) may be utilised by third party applications (mailchimp, Valdhub) in order to send emails or utilise in clinic strength testing.

What are your data protection rights?

The right to access – You have the right to request Noreside Physio for copies of your personal data.

The right to rectification – You have the right to request that Noreside Physio correct any information you believe is inaccurate. You also have the right to request Noreside Physio to complete information which you believe is incomplete.

The right to erasure – You have the right to request that Noreside Physio erase your personal data, under certain conditions.

The right to restrict processing - You have the right to request that Noreside Physio restrict the processing of your personal data, under certain conditions.

The right to object to processing – You have the right to object to Noreside Physio’s processing of your personal data, under certain conditions.

The right to data portability – You have the right to request that Noreside Physio transfer the data we have collected to another organisation, or directly to you, under certain conditions.
